MOS Score when you have only 1 cisco router with IP SLA?

Is it possible to get MOS score when you have

PBX->CiSCO->IP Phones (difrent location)

There no problem to ping the IP Phone from the cisco or the IP PBX.

I am afraid you cannot get the data you are looking for. In order to measure MOS value using IPSLA Voip UDP Jitter operation, you need to have IPSLA responder running on a target. If it is not serving on PBX or IP phones, the operation running on Cisco router cannot succeed. Also, if you really want the direction PBX->Cisco, PBX need to be Cisco and IPSLA capable in order to execute the operation.
