Dell disk status alert email format

I have configured UnDP to poll dell disk status and it working fine. but i have a question related received Alert email format. I want email alert like disk number and status in tabular format. currently i have following configured.

And i am receiving email like: Dell disk array status is online, online, online, online, foreign on Server.xyz.com

It looks like the disk number is not defined in the poller, so that data is not in the database.
