How to get port ID in UDT alert message

I would like to have an alert sent when a Watch List item is seen on my network again. I found the canned alert for his, but I am unable to determine how to include the name of the swithc and the port number in the allert message. I cannot find a SQL variable for this.

Can anyone help? Thank you.
