I know you can uncheck items from the SmartMonitor list, but how do we delete items? There are some custom services that are no longer used, but at one point got added to the SmartMonitor list. We'd like to clean this up and delete them.

when you have clicked on the smartgroup folder icon in the left pane, you should then be able to click Edit and edit the properties or mass edit that smartgroup. Let me know if this helps.
The SmartMonitor list is a list of known/recognize monitors that can be configured to be automatically enabled when they are discovered during the Discovery process. In the Device Discovery wizard there is a link for "Change SmartMonitor Settings". The only thing you can do here is enable/disable monitors from automatically being included in the scan. There are some old monitors listed there for customer services that we want to delete as they have the wrong names and are no longer used.
Try this:
click on the Devices tab
click Add new Smartgroup
From this page you should be able to edit the Filter rules.
