Monitor VoIP cloud service

We have a provider  based VoIP system. We do not have access to the gateway equipment so the current SW VoIP product is of no use.  What we would like would be a monitor that would periodically attempt to place a call and send an alert if there is no dial tone, the call fails etc. Is there a device that acts like an automated deskphone that can actively alert if there are phone system problems?

this functionality is not implemented, you might create Customer Feature Request

The main intent of IPSLA operations, that you probably mean when you note there is no access to GW from you side, is to monitor quality of the lines, to simulate the traffic by active elements of the network.

Simulation of call from phone device to phone device is not implemented, the substitute for this functionality are for instance Voip UDP jitter, ICMP path jitter, ICMP path echo.
