Need custom SAM template that duplicates the Appinsight for AD template

I need to create a custom SAM template to duplicate the Appinsight for Active Directory template. Since the components in the Appinsight templates are not available in the Component Monitor library, I can't build the template that way. I have been able to add or discover all of the component types except the "LDAP monitors".

Does anyone know how I can find and add those monitors?
