Don't know if it was just me but the part where we rate the items with numbers... no matter what number I clicked on next to an item it kept renumbering everything from the top to bottom 1,2,3,4,5 like that ie. I couldn't re-arrange the numbers.

When you choose the number, it should reorder the items for you according to the number you chose. Were you able to submit?
