When a user fills out the IP address request for, we require additional fields, such as hostname. When the e-mail with the list of approved IP addresses is sent to the requester, only the assigned IP addresses are included, with no information as to which host the IP belongs. I would like to be able to choose which fields are included in the e-mail, or at least have any required fields included.
Right now only the following is sent in the e-mail with the address approval e-mail:

Details of the request are listed below.
Approved IPs:
- 10.5.31.11
- 10.5.31.12
- 10.5.31.13
- 10.5.31.14
- 10.5.31.15
