Add timestamp and user to alert comments, and retain comment history

Right now, when someone adds a comment to an active alert, the user ID and date/time are not included in the comment. This makes it difficult to know when and by whom the comment was made without manually querying the AuditEvents table. Please add the user ID and timestamp to comments.

Also, a new comment overwrites any existing comments on an alert. Please retain old comments so that we can see the history for an alert.
