WPM - Element 'span' was not found

Due to SSO we configured service accounts for two of our homegrown applications specifically for running WPM transactions.
The service accounts have WPM rights, have been added as accounts able to run the transactions, and work fine after being created or re-saved.
One of the WPM transactions works the first time and has had no issues, the other which was setup identically fails the next day with the message "Element 'span' was not found"
Randomly it will fix itself and run fine and then come back with no explanation.
Nothing in event viewer.

I have seen this a coupe of times and when the issue happens, i log onto internet explorer immediately at the player location to see the exact error in browser. I generally find it is proxy server or incompatible version of internet explorer. You have to make sure the credential being used has access through all proxy servers.
