From the errors that you've provided, it looks like the Provider for the EVA isn't online or properly responding to requests.  There are a few references to help you test and the HP Command View EVA Software.

From the HP SMI-S Support Matrix, you should run Command View 4.1 or later (though we normally recommend the latest version available).  Beginning on Page 316 of the Storage Manager Administration Guide, we give step-by-step instructions on how to install the HP EVA Provider.  Once you confirm that the provider works as expected, the data should collect without issue.
