Also, those systems will report the package as Installed, even though it's NotApproved. From the Third Party Updates view, select the update and use the Computer Summary tab to see the machines that have the update installed.

Just a follow-up.. most notably this scenario occurs when WSUS is reinstalled on an existing Patch Server. Because the WSUS registry key gets deleted and recreated, it then lacks the permissions that the Patch Manager installer adds to that key when Patch Manager is installed.
