What are the Reset Conditions that you have setup? Are you using Reset when the Conditions are no longer true?

If it is showing up as [Unknown] then it is only monitored by ICMP only. Orion will not resolve the IP Address to a Hostname as well if Orion is monitoring only via ICMP, but you can Edit the Node to change the Name.

What I would do is take a look at the disk that the trace log is writing to. The STIG rule states that if the log cannot keep up, the SQL Server shuts down.
