Again, sorry for the delay. Under the Additional Settings tab try unchecking 'Permission Required.' Under the 'Access' tab leave all three boxes at the bottom unchecked. We were able to reproduce your issue, and this combination of settings worked for us. Please try that and reply back here if it still doesn't work for you.
