I had the same problem when running the .MSI directly. Solved by running the .MSI as administrator by browsing to the file using an elevated command prompt. I run SCOM 2012 on a Windows 2012 R2 with the lowest UAC setting.

Our problem was that neither the PDF`s nor the HTML emails was sent. The solution: Instead of using an "Active Directory" account (domain\user) on the "Orion Web Login"-tab, we used a "Orion Account". (Create a new account named "Reports" or similar)

This happens here aswell, but only with certain sensors. Those 3 for example, I have tried disabling them several times now, but after a couple of minutes they are "re-enabled". It happens for the same sensor on different nodes. The nodes have not been un/remanaged and we do not have an unmanage scheduele.
