This one is really baffling. Just for kicks could you try changing the "Platform to run polling job on" setting just for this one instance of the assigned application that's not working and see if that changes the behavior?

It's likely the license is activated by not assigned to the polling engine. You can verify this by going to [Settings > All Settings > License Manager]. There you should find your APE license. Ensure it's assigned to your APE.

This sounds like a permissions or firewall issue. Ensure that you can connect to that host from the Orion server via WMI using those same credentials with wbemtest.exe
