Alternatively you can use AppInsight for SQL included in SAM 6.x which will return a list of the top XX most fragmented clustered and non-clustered indexes. You can even exclude indexes that are smaller than a particular size to eliminate indexes that fragment quickly.

Yes, if you were placing this resource on say a Summary view which has no macro information it can pass than SWQL would be the way to go.

Note that AppInsight for SQL should not be assigned to the SQL Server itself when operating in a cluster. AppInsight for SQL should only be assigned to the Cluster itself via a node that represents the VIP, not each cluster member individually.

The only Windows account that has adequate default permissions to allow SAM to monitor is the local or domain administrator. There's certainly nothing more frustrating than spending hours creating something, and then losing it when you go to save. I suggest you safe often and if this persists, open a case with support so we can troubleshoot the issue further.

SNMPv3 should make no difference. You are very much so on the right track. Worst case, if the OID in fact existed you might need to change the SNMP GET to a GET-NEXT, but since the OID doesn't exist at all that's not going to help any either.
