Correct, this is a function of the Advanced Alert Manager. You can define in the application template thresholds but all alerting criteria is handled within the Advanced Alert Manager.

I can't say that we've seen this issue before. Please enable Debug Logging on the AppInsight for SQL application where you're seeing this issue. Should the problem occur again, please gather a SolarWinds Diagnostic immediately. Then open a case with support. We will need this information to properly diagnose the issue.

Changing it where I described above will change the threshold only for this particular instance. Modifying this value from within the template itself will change this value for all instances of SQL. You can modify the template by going to [Settings -> SAM Settings -> Manage Templates] as seen below.
