I was speaking of the last mile to the hosted location for the SaaS provider. I could shut the provider down for part of a day as well as all their customers.

Good points here...on the topic of patching though...you only specifically talk about OS patching. Application patching as well as related libraries is just as bad and you must be diligent in keeping those patched as well (java, flash, etc.). Ah...knee jerk reaction. Account provisioning is like anything else...it has to be documented and backout methods provided when changes are made. I have seen that same thing bite hard at more than one company...sometimes they disable to see who screams and other times they just rip off the band-aid.
