Sounds like a name resolution issue on the main poller system itself if you are still getting the old IP. Did you try to flush dns and cleat the netbios cache?

Server 2012 is indeed presently supported. SQL Server 2014 is support is targeted for NPM 11 (presently in RC.)

With the new web-based reporting in 10.6, you can create a report with existing resources, filter for top N, and set relative time periods pretty easily.
