Do you have NCM? There is a default report called "Cisco 3750 Stack - Physical Entity" which provides most of the info you require (except for which switch is master, member, etc but that could probably be added by editing the report)

I would at least hope for a facility to import a list of Session names/IP addresses so that I don't have to do one at a time - but if this could ultimately tie in with the nodes in our NPM environment, that would be ideal
