You can alert on HSRP flips via traps or Syslog. As for interface, if a secondary router takes over, all interfaces on the affected subnet are affected. What do you want to report on interface? Are you talking about the physical interface or the layer 3 interface?

It is possible. If you look at the DB, there are probably 2 entries with the same IP, but different node IDs. Delete the one you don't want.

Did you run SolarWinds Diagnostics before the rollback? That should give you a list of anomalies that would point you in the right direction.
