I believe UPS polling is done via Cortex. If SAM template's correct, then there's probably a problem in the Cortex. Are you on the latest Orion version? And yes, I agree - open a ticket for that.

From the SNMPWalk result, were you able to see indexes for your interfaces? I'm thinking since device is using SNMP version 3 it might be able to see the SysOID but not the information under it. I believe at this time, there is no way you can add notes to the Node Details web page unless you have Node admin rights and use Custom Property.. This may be because the Node details are stored on the Nodes table and if there's a need to modify or update the node, it will automatically right to the database.
