how are you polling your servers ... SNMP ? if you are i believe the only way to see DISK IOPS is polling with WMI - I am sure if i am wrong someone will correct me

Alert on Nodes that stopped responding to SNMP here is a nice thread with some solutions and tweaks .. everyone kinda does it a little different Alert me when a managed node last poll time is 10 minutes old

Opened a case with support - case number 588106 We ended up removing NTA 4.0.1 and reinstalling it and then SAM installed fine after that with no issues.
