You could use the Linux script monitor in APM, or what would probably be easier would be to monitor the java process via SNMP. 

Can you post screenshots of the resulting output from SQL Management Studio? The SQL query itself may be helpful too, but honestly the best way to diagnose the problem for me has been to look at the resulting output from the query itself in SQL Management Studio.

Is it possible you posted the wrong case number? I looked up case #380905 as posted above and this case isn't regarding SAM.

There currently is no concept of template variables in APM as you describe. We are looking to make improvements to application template editing in APM in the future and will consider the use of variables as a potential solution to ease this process. 

Can you post a screenshot of the query result as seen in the SQL Management Studio?

An alert configured similarly to the image below should notify you if there is more than a single instance of that process running simultaniously.
