what versions are you on (core, npm, sam) ? There is evidence there is a problem with Platform 2015.1.2, SAM 6.2.1, and NPM 11.5.2 regarding alerts not firing on time (6 hours later recently for us), firing on un-managed servers, un-managed servers not re-managing on time.
