For anyone coming across this, check to make sure the shared mailbox is configured to "allow messages from outside the organization" (Office 365) or from unauthenticated users (on-premises).

@"danielleh" - thanks for the reply. I was using Firefox to access the page; when I didn't see the form you showed, I restarted Firefox with all extensions/add-ons disabled and still didn't see it. After I cleared cookies+cache, I could see the form, and have registered. Thanks for your help!
