Availability and uptime for service/process being monitored by SAM

Hi,

See outline below of the Great Plains ERP se - on the ServerOne server - the following windows services are being monitored with SAM

I would like to create an availability/uptime report of services being monitored with SAM within a specific time frame for a 30 day period. I have shown below how I would like the report from the NPM/SAM environment.

I would highly appreciate your assistance to accomplish this report.
