I have been getting some pretty slow responses from SolarWinds and it was advised to install SAM AppInsight for SQL to see if there were Database issues.
The below image shows some very high fragmentation and this is after I run the database maintenance wizard and after we run our own de-fragmentation.

These indexes are a bit of a red herring and are not the root cause of your performance issues. What you are seeing are indexes for tables that were essentially just created recently. Orion creates a new tables daily to store that days results. This also means that nightly database maintenance has not yet been run to defragment the indexes, which is why the index fragmentation is so high. This is fairly benign though since these tables are relatively small, and therefore do not impact performance. While small, they are still larger than the default 1MB index size ignored by AppInsight for SQL. My recommendation would be to increase the "Minimum Size (KB) of indexes to Retrieve" from 1024 to 10240 (10MB) which should remove these indexes from being monitored. Nightly maintenance will rebuild those indexes at the end of the day ensuring they are properly optimized once Orion is no longer adding additional data to that table.
I believe there are multiple ways to navigate to these settings. Here's how I did it:

I ticked the check box for our Orion DB monitor, clicked Edit Properties, expanded the Top Indexes for Database SQL Table Monitor and changed the threshold for only that SQL instance by clicking the Override Template button to the right of the Minimum Size (KB) of Indexes to Retrieve threshold level.
