That particular model does not support the CISCO-STACK-MIB mib which does provide identifying information from the OID for Primary & Secondary.
Although this Cisco 7206 shows both power supplies, I can alert if either fails, however I can't tell which is primary and which is secondary. I think you could do the same with alerts.
I can generate a different Alert for the first and second PSU using Row ID

I looked CiscoStackMIB
MIB is supported by Cisco7600, but is not supported on Cisco4500 (((
I use 18.104.22.168.22.214.171.124.126.96.36.199.3 for the first power supply, and 188.8.131.52.184.108.40.206.220.127.116.11.3.2 for the second power supply. We are in load sharing mode. Seems to work. I haven't turned one off to test though.

I think the second has to be Get only because it's pulling from a table.
