We have number of cisco 6500 series router/switches.I want to monitor all modules status when any module is having any problem it needs to send an email.

Syslog will not give you the ability to monitor statistics over time so there wont be a baseline to compare to.
I would also like to do what the original poster asked. I have done a lot of MIB walking/browsing and hunting on the web for those OIDs but can't find them.
Any hints?

As I understood, we need to use two OID 1.3.6.1.2.1.47.1.1.1.1.13 and 1.3.6.1.2.1.47.1.1.1.1.11 .
However, we have multiple modules in CAT 6500. How we can get serial and name for each module? Is it possible?
And finally, Is there possible to find "Node name/IP" that has THE "Serial number"?

Try using this UnDP for getting an inventory of your modules. It should give you the slot number, software version of each module, each module's model number and finally each module's serial number.
You can also try this UnDP for pulling the switch's serial number:
It's a few years old but it probably still works.
