I instructed QA to try to reproduce the issue from the information you provided. Here are the steps that they took:
1. Install Orion 9.0 SP2
2. Install VoIP 2.0 SP3
3. Install Netflow 3.0 SP4
4. Remove NetFlow
5. Remove references from the registry
6. Reboot VM
7. Reinstall Netflow 3.0 SP48. Reconfigure NetFlow.
We executed these cases for both SQL 2005 and 2000, but were unable to reproduce the issue.
