TCPIP port monitor

I am new to SAM and need to create a TCPIP port monitor. Sitiuation is I have a standalone server (old OS and installing the SAM agent is not possible). I need to monitor application X which uses port 2500. So I want to create a monitor that will alert when port 2500 goes down if I can monitor the application as well that would be great. But the only polling method I have available is SNMP.
