Creating a Custom Application Template for SQL

Hi

I am struggling to create a Custom Application Template for SQL on SAM and need to do the following.

AppInsight for SQL contains 100 SQL components that can be monitored but I don't need to receive metrics on all 100 components and hardly any of them can be DISABLED.

I then attempt to create a 'New Template' and when searching for any of the SQL components from Insight there aren't any available.

So if I want to create a Custom SQL Template which includes components from 'SQL Agent Job Info' to 'Missing Indexes' how do I do this?
