SRM Blok Provider is Unknown (Unreachable)

Hello Everyone,

anybody know why this happen?

I already do steps from

SRM: Enable a HP P2xxx / MSA onboard provider - SolarWinds Worldwide, LLC. Help and Support

and

Storage Array is unreachable - SolarWinds Worldwide, LLC. Help and Support

but.. stil not fixed...

*) currently use latest SRM 6.6
