Application monitor will pull the PID table via the Host-Resources MIB and allow you to select any process you'd like to keep track of (CPU/Memory used/Availability, etc).
ipMonitor might be more what you are looking for, as it will dig further down into the health of certain services and may even tell you the link info around your db connections (I'd need to check to be sure). With ipMonitor you would also be able to select the named service that you want to monitor as opposed to the process that it ends up calling.
