How to monitor SQL 2012 in SAM with what type of permissions?

In my environment, I have joined my SQL server with Active Directory and want to monitor Apinsight in SAM for SQL.

I don't want to monitor with Administrator and i have created account in AD and want to monitor with that account by giving some sort of permissions as recommended by SAM.

I am literally confused on WMI credentials and in SQL account. What type of credentials and permissions required to monitor Apinsight in SQL and its joined with AD?

How are you attempting to monitor the SQL application? If you're using AppInsight for SQL then it should be as simple as clicking "List Resources" on the node and selecting the instances you want to monitor. Clicking on the SQL instances from the "All Applications" resource will prompt you for credentials needed for monitoring the SQL instance. This account can be the "sa" or equivalent, or you can create a dedicated  least privilege user account for monitoring SQL by following the instructions described in the KB article below.

I am confusing in WMI credentials and in SQL credential.

And i am getting following message in SAM.

Unable to establish SQL connection, check credentials. A network-related or instance-specific error occurred while establishing a connection to SQL Server. The server was not found or was not accessible. Verify that the instance name is correct and that SQL Server is configured to allow remote connections. (provider: Named Pipes Provider, error: 40 - Could not open a connection to SQL Server)
