It may be that your Process is known by a slightly different name?
Try using the "Real-Time Process Explorer" or the "Service Control Manager" from the Summary view of the device, locate the SQL process or service from that device and then use the "Start monitoring".
Service wise it will be something like "SQL Server (<instance name>)" or process "sqlserver.exe".
The wizard will then let you add this to your existing template, just remember to remove the one that is failing (or disable it) from the device's template.
