Retake voucher

Hi, 
 
 Is there a delay in receiving the retake voucher? I have received the email saying I could get one but I have recieved no response when requesting it.

There should be a chart/form you fill out after you request the voucher. 
 We did hit a bit of a delay as we had a team member out unexpectedly. We are working to catch up on issuing vouchers. 
 We appreciate your patience.
