Hey Supersizeal , would you be willing to share yours? Below is what we're using but would love to see what others came up with.
you can always build a new one with the fields you want by navigating to the service desk> service catalog and clicking on the plus button to your top right. you input fields you would want and build a process behind it.
Let me know if you have any worries concerning that
I started building a new onboarding form but I have a couple of questions that I can't seems to figure out.
1. How do I remove the * of a custom field?
2. How do I just give HR access to this form in Service Catalog only ( no other department can see it)?
3. How do I setup approvals and notifications?

Hey Supersizeal , happy to help!
1. To change the field from required to not required, Go to the Service Catalog item with the field you'd like to update and open it. Hover over the field and you should see ellipses (...), click that then "Edit", you'll see a warning screen of sorts that will tell you if that custom field is used in any other service catalog items. This is important as any changes will affect all of those. If you'd still like to edit this field, click "confirm" and on the next screen you should see a check box for "Mandatory". Save your changes with "Update".
2. To do this you'd use the Site and Department fields found in the Details portion of your catalog item. See my screenshot above but note, we didn't filter ours. The site and department fields are populated either manually or by what was included when users are synced.
3.For this, you'll use the Process section of the catalog item. This can be seen next to the details tab in my screenshot.
