MFA invalid passcode

This is common for a few users as well as myself (master of SU)
Invalid MFA passcode; user: "XRZ@CA"; entered passcode: "**********"; computed passcode: "**********"

Using Duo, 
I'll change from Edge to Chrome, this works about 50% of the time
Wait a few minutes, close browser, and/or open new tab, and use new TOTP

Annoying for me, but very difficult for average users, and I'm not sure which Auth App they're using

Is there any, diagnostics to try to catch a log to determine the fault?
