We have noticed an older patch that was declined, but we are unsure when it was declined and would like to see that date. Is there a way to look up that information?

If it was done through Patch Manager - and the Task History event for that change hasn't been cleared - you can run a report or filter the task history to find it. In this case, I went and Declined a NotePad++ update that I had published (but it shouldn't be shown any differently for regular Windows updates); you can see the Operation listed in the screenshot below for that first line as "decline_updates_wsus_server" and there is also a Completion Time column that shows when that happened.
Not sure if this info will help you, though, if the action was taken on a previous install of Patch Manager!
