Query for Last Week, Last Month, Last Year Node Availability

Trying to put together a widget for a dashboard for Site (Node) Availability for Last Week, Last Month, Last Year. I know there are Reports that do this but getting those in to a dashboard and not being 3 separate tables seems impossible .

Example:

Site(based off switchport Availability)    Last Week                Last Month                  Last Year

Avon                                                            100%                      100%                        99.89%

Any cool formatting past that would be great. Threshold color change for some percent, say < 95%.
