Move the API poller out from under the node and move it to an application Monitor Template. Also Allow multiple servers to use one custom API monitor

GRoup the API Polling with the application Monitors and allow multiple nodes to be polled by a API poll.  We use the API polling to check the status of several servers behind a load balancer.  I have to create new names for each API Poller instance for each node even though they are identical.
