Hyperlink on custom tile

Dear All,

We have created the tile view according to application used in our organization. Refer Attached image Appview.

Tile view shows only count of node and active alerts so we have create a details page of all our application whick shows Top XX utilization of CPU/disck/interface/Memory,

For user friendly navigation we want to add hyperlink in all tile view, when clicked on tile view the corresponding application details page should open, Refer attached image Appview marked in red colour.
